When we need medical care and we cannot give our consent because we are unconscious, there are two ways in which we can still express it in advance and both are compatible: by appointing a representative or by writing a document that can be consulted by doctors.
The document is called by different names and in it we write the medical acts we accept or refuse.
It is advisable to sign it in front of a notary, so that it has more legal importance.
In this document we can name representatives or not. In the case of appointing a representative, the document should serve him as a reminder of what we want and what we reject, but maybe he can decide something different from what is written.
In addition to this document, it is advisable to carry in our pocket, along with the documentation, a card or sheet with a summary or copy of it, for the rare case that the doctors need it and do not have access to the document in the central registry.
The JW.org people carry a copy of it folded into 8 parts, so that it ends up the size of a credit card.

When the patient's will is clear (the patient is an adult and is conscious and serene, or has a ADD) the physician cannot impose a treatment or diagnostic test. However, when these circumstances are not present, there is a greater likelihood that the physician will impose his will, for example, when the patient is a minor and/or the physician considers the patient's representative to be ‘unsuitable’, ‘questionable’, ‘debatable’, ‘unsuitable’. In the case of minors, the doctor's opinion always prevails and is supported by the judiciary. Perhaps only if the representative is a doctor will it be possible for him to oppose the doctor's will, which is why it is advisable for adults to make a ADD.
That is, if a doctor decides that a child needs a transfusion, the judges support him. The parents' opinion is not taken into account.
In the Advance Directive, in addition to explaining that the seawater was prescribed by the French Social Security, I would add that doctors do not have the right to claim ‘conscientious objection’ to refuse to operate on someone who refuses transfusions. This statement, apart from having legal backing, is explained by bioethics committees.
Text on the issue of transfusions:
I do not want to receive transfusions (blood or blood products from other people or animals). Instead of transfusions or saline solution, I want to be injected with isotonic seawater (preferably Quiton.es brand).
Sea water (diluted) was prescribed by the French Social Security until 1980, by drink or injection, without contraindications, as stated in their vade mecum (‘Dictionnaire Vidal’) which can be consulted at martin13.com/en/090.html, so it cannot be claimed that its use is against good medical practice, as this would be tantamount to saying that the Social Security in France in 1980 was massively carrying out bad medical practice, as sea water was massively injected (and since the beginning of the century). Ignorance of this does not justify calling the medical use of sea water as ‘malpractice’. The Quinton.es brand has the authorisation 7627-PS of the Spanish Association of Medicines and Medical Products for the manufacture of medical products.
